Chaperone Agreement

(See attachment) To be signed by the Chaperone responsible for the child during the Summit.
Please remember that Chaperones are required to be at least 18 years of age (though we recommend that they be 21 or older), are required to be on site at the Summit throughout the entire event, and are responsible for personal and group behavior.

GENERAL RULES AND REGULATIONS FOR PARTICIPANTS UNDER 18 YEARS OF AGE

The following National Youth Venture Summit Rules and Regulations apply to participants who are under 18 years of age. If you are under the age of 18, you are required to be accompanied by adult supervision to attend the National Summit.

The Youth Participant, his or her parent(s) or legal guardian(s), and the Adult Ally/Chaperone understands that any noncompliance with these terms and conditions is grounds for immediate dismissal from the Summit regardless or age or adult authorization, and that such dismissal is at the Youth Participants’ or their sponsoring agency’s expense, which includes forfeiture of any financial assistance awarded by Youth Venture.

Please share this information with your parent or guardian. Both parents and adult supervisor should make copies of the form for their records.

Terms and Conditions of Participation: We (the Youth Participant and parent/guardian) understand that the terms and conditions participation in the Summit of Youth Participant and Adult Allies/Chaperones are as follows:

• The possession and/or use of weapons, alcoholic beverages, and illegal drugs is strictly prohibited.

• Violence of any kind will NOT be tolerated and is strictly prohibited.

• Any behavior that contravenes university campus rules or polices or is otherwise inappropriate is strictly prohibited.

• Each Participant must pay the National Youth Venture Summit registration fee prior to attending the Summit. Visit: www.genv.net/en-us/summit for more information about Summit costs and method of payment.

• Each Youth Participant must be present at ALL Summit activities.

• Each Youth Participant and their Adult Ally/Chaperone will follow the instructions of Youth Venture representatives during the Summit.

• Male Youth Participants will not be allowed in any female Youth Participant university campus residence or vice-versa at any time unless supervised by an Adult Ally/Chaperone.

• Youth Participants are not to leave the university campus at any time without specific permission and authorization from their Adult Ally/Chaperone. Youth Participants are also not to leave any Summit event without notifying a Summit facilitator or Youth Venture staff member.

• An Adult Ally/Chaperone has ultimate responsibility for his or her Youth Participant at all times. Summit staff will be present and available during Summit sessions and activities, but responsibility for attendance and behavior of the Youth Participant remains with the Adult Ally/Chaperone. They are responsible for signing out (and checking back in) with Summit staff whenever his or her Youth Participant leaves the Summit, and on the final day of the program.

• Attendance, participation, and punctuality are required at every Summit activity.

• The Adult Ally/Chaperone will ensure the timely arrival at the designated location at the beginning of the Summit. Each Participant must arrive in time to participate in the activities that they have registered for. Each Participant registered for the National Youth Venture Summit must be present by Friday, June 26th, at 6pm local time.

GENERAL RULES AND REGULATIONS FOR PARTICIPATION, CONSENT AND RELEASE FOR PERSONS 18 YEARS OR OLDER

The following National Youth Venture Summit Rules and Regulations apply to participants the age of eighteen (18) years or older. All participants, regardless of age or adult authorization must adhere to these rules; failure to comply is grounds for early or immediate dismissal from the program at the participant's and/or their sponsoring agency's expense, including the forfeiture of any financial assistance awarded by Youth Venture.
