Henry Steet Settlement
Henry Street Settlement can help you apply or recertify for:
1) Child Health Plus
(covers children from birth to age 18):
Covers well-child care, immunizations, emergency care, prescriptions, x-rays and more.
2) Family Health Plus (for adults 19 to 64):
Comprehensive coverage, including primary care, hospitalization, prescriptions and more.
3) Medicaid (all ages):
Free health coverage to individuals who qualify.
Documents needed to enroll:
Proof of address: phone, utility or cable bills; postmarked envelope or rent statement (within the last six months)
Proof of income: pay stubs; letter from employer; unemployment benefits statement or letter; or letter of support (most recent)
Proof of identity: passport; permanent residence card (green card); birth certificate; or employment authorization card. Everyone must bring his/her social security number.
Proof of resources: current bank statement or bank book
